Amida Care is hiring a Care Coordinator - TGNB Health to lead clinical care coordination and oversight for transgender and non-binary members, including case reviews, inpatient census monitoring, referrals, advocacy, psychosocial assessments, education on gender-affirming procedures and hormone treatment, and community outreach. The role requires a Licensed Practical Nurse, with a Registered Nurse preferred, and includes one year of transgender-community experience and one year of HIV/AIDS, medical, or chronic-care experience as preferred qualifications.
