Description
The Care Manager coordinates care for patients and their family members and caregivers, using motivational interviewing and panel management to improve access to and adherence with primary care. The role develops care plans for high-risk patients, coordinates referrals and hospital follow-ups, tracks discharged and preventive-care patients, and performs outreach in primary care sites, homes, hospitals, and neighborhoods. A high school diploma or equivalent and 1–3 years of relevant experience are required; a relevant bachelor's or master's degree, health-home or care-management experience, a New York State driver's license, and bilingual Spanish are preferred.
