Description
The Care Manager RN will provide comprehensive care management for high-risk, chronically or acutely ill patients, including assessments, care-plan development, coordination with patients, families, caregivers, and multidisciplinary teams, home and facility visits, referrals, and end-of-life discussions. The role requires at least five years of clinical and case-management experience, an active Ohio RN license, an Ohio driver’s license, and the ability to travel throughout the service area; CCM certification and a bachelor’s degree in nursing are preferred.
