Description
The Care Management Supervisor reports to the Director of Care Management and oversees Care Managers while ensuring client services meet regulatory, programmatic, and agency standards. The role coordinates outreach, eligibility and acuity assessments, plans of care, billable services, documentation, and linkages with residential treatment, foster care, and preventive programs. It requires a bachelor’s degree in a relevant field, at least three years of human services, health care, or related experience, and strong organizational, communication, and project-management skills; a master’s degree or nursing qualification is preferred, as is supervisory experience in Health Homes or Medicaid programs and billing experience.
