Description
The Care Manager oversees the client care experience for Right at Home, conducting assessments, creating and updating individualized care plans, completing client and caregiver supervisory visits, educating and supporting caregivers, coordinating with families and office teams, documenting services, and ensuring compliance with HIPAA, regulations, and company standards. The role requires local travel to homes and community settings, a valid driver's license and reliable transportation, and participation in designated after-hours care-management coverage.
