Description
The Care Manager supports individuals in making informed decisions about long-term services and supports, coordinates formal and informal caregivers, conducts person-centered assessments and interviews, completes client documentation, monitors care plans and service delivery, and collaborates with interdisciplinary teams. The role serves the New Britain/Hartford community, requires a bachelor’s degree and at least two years of direct case management or related health-care or human-services experience, and pays $50,000.
