CASES is hiring a full-time Care Manager to provide person-centered care management, care coordination, health promotion, transitional care, referrals, and support to members and their families. The role uses motivational interviewing, trauma-informed and recovery-oriented practices, and health information technology, with at least 80% fieldwork and a Monday-Friday schedule from 9:00 AM to 5:00 PM. The position pays $50,000 per year and is based at the New York office, with flexible work from home or another suitable space after reporting to the office.
