Community Care is hiring a full-time Case Manager/Care Manager for its Family Care Program in Milwaukee, Wisconsin. The day-shift role manages Family Care Program members, completes clinical assessments, develops member-centered care plans, coordinates care, provides education, monitors service quality, and requires regular local travel. Applicants need a bachelor's degree in Human Services or a closely related field, at least three years of recent experience with older adults or persons with physical or intellectual disabilities, and current long-term care functional screener certification eligibility; three years of community nursing experience is preferred.
