FreedomCare is hiring a field-based Care Manager to support home-care operations in Indiana, with frequent travel to patient homes in the Terre Haute area. The role assesses client needs and environments, evaluates caregiver fitness, conducts home orientations and initial assessments, supervises ongoing cases, trains caregivers and patients, participates in interdisciplinary meetings, and improves care procedures. Candidates should have at least two years of experience with frail or elderly disabled adults and caregivers, preferably in medical or case management, and should be able to work with ADL/IADL assessments, data entry, Salesforce, Microsoft Office, and communication tools. The position offers a base pay range of $22.00 to $26.00 per hour.
