FreedomCare is hiring a Care Manager for its home-care operations in Connecticut. The role assesses client needs and home environments, conducts orientations and initial assessments, supervises caregivers, provides training, participates in interdisciplinary meetings, and improves care procedures. The position pays $50 per completed field visit, covers multiple Connecticut counties, and requires travel within Indiana for reimbursement. Candidates should have at least two years of experience with frail or elderly disabled adults and caregivers, preferably in medical or case management, and should be able to perform ADL/IADL assessments, use Salesforce and Microsoft tools, and hold a valid driver’s license and insured vehicle.
