Elm & Oak Health is hiring full-time remote Care Managers to serve members across the Albany and Capital Region in a Health Home care management role focused on underserved populations. The position involves conducting bio-psycho-social assessments, developing person-centered care plans, coordinating services with providers and community agencies, maintaining regular member contact, and completing timely documentation. Candidates must be willing to travel throughout the region, have experience working with adults, and meet one of several education-and-experience pathways or equivalent credentials such as CASAC. The role requires strong communication, organizational, computer, and independent work skills, along with a NYS driver’s license and reliable transportation.
