Description
The Case Manager provides comprehensive assessment, care planning, home and facility visits, community-service coordination, crisis intervention, advocacy, documentation, and ongoing monitoring for older adults and adults with disabilities. The role requires a bachelor's degree in social work or a related field, at least one year of experience working with disabled or older adults, and experience in community health care delivery; bilingual English/Spanish is preferred. Compensation is listed at $75,000 to $78,000 annually.
