Description
The Case Manager conducts comprehensive and ongoing assessments of residents, develops client-centered Community Living Plans, coordinates community and medical services, provides advocacy and crisis intervention, documents care activities, and collaborates with clients, caregivers, and health-care providers. The role requires a bachelor's degree in Social Work or a related field, at least one year of experience with disabled adults or frail elderly, case-management experience, computer literacy, and the ability to lift up to 25 pounds. Bilingual ability in English plus Cantonese, Mandarin, or Spanish is preferred, and the annual compensation range is $70,304 to $75,000.
