Molina Healthcare is hiring a remote Care Manager to provide long-term services and supports (LTSS) care management and care coordination for high-need members. The role includes comprehensive assessments, waiver enrollment and disenrollment, care-plan development and monitoring, service authorization, benefit evaluation, interdisciplinary team collaboration, motivational interviewing, and assistance with barriers to care. The position requires at least two years of relevant healthcare experience, including LTSS and care-management experience, and may require local travel throughout Dane County, Wisconsin.
