Description
The RN Care Manager will organize and coordinate high-quality, patient-centered care for patients identified through high-risk registries. Responsibilities include developing individualized care plans, coordinating care across settings, monitoring progress in an EHR, providing proactive education and support, and maintaining relationships with patients, families, and caregivers. The role requires a BSN, current Ohio RN licensure, at least three years of nursing experience, knowledge of chronic conditions, strong assessment and communication skills, and the ability to work independently; EPIC knowledge and a leadership role are preferred.
