Description
The Patient Care Navigator serves as a link between patients, families, and the healthcare team, coordinating care, scheduling appointments, supporting chronic-disease and preventive health maintenance, managing transitions, completing quality-gap interventions, and documenting interactions in an electronic health record. The role works with multiple payers, primarily Medicare Advantage, collaborates with providers and interdisciplinary teams, reviews population-health data, and may lead clinical projects. It requires a high school diploma or GED, completion of an accredited Medical Assistant program with certification, and 3–5 years of relevant healthcare experience; LPN licensure and EPIC experience are preferred.
