Description
The Patient Care Navigator serves as a patient-care coordinator for Premier Physician Network, guiding patients through appointments, specialist referrals, transitions of care, medication reconciliation, and quality-gap closure. The role coordinates with providers, RN care managers, multidisciplinary teams, and community organizations; reviews population-health and payer data; documents interactions in an electronic health record; and may lead clinical projects. It requires a high school diploma or GED, completion of an accredited medical assistant program with certification, and 3–5 years of relevant healthcare experience, with preferred LPN or medical assistant experience.
