Description
The Care Partner coordinates safe, efficient, and patient-centered transitions of care for hospitalized patients. The role evaluates patients for home-based and post-acute care, supports discharge planning, arranges services such as home health and hospice, educates patients and families, manages referrals and documentation, and contributes to hospital quality and readmission performance. It is an in-person role requiring bedside engagement and interdisciplinary collaboration, with an associate's degree required and no prior experience required.
