Silverchain is hiring a full-time permanent Care Partner to support older Australians receiving government-funded aged care services in the Blacktown region. The role combines community-based client visits with the flexibility to work autonomously from home and involves managing a caseload, conducting assessments, developing care plans, monitoring budgets, coordinating services, supporting onboarding, building relationships with clients and stakeholders, and advocating for positive health and wellbeing outcomes. The position requires strong communication, empathy, problem-solving, organisational, and teamwork skills, along with a current driver's licence and reliable vehicle with appropriate insurance.
