Silverchain is hiring a full-time permanent Care Partner to support older Australians receiving government-funded aged care services in the North Sydney region. The role combines community-based client visits with the flexibility to work autonomously from home and involves managing a caseload, conducting assessments, developing care plans, coordinating services, monitoring budgets, supporting onboarding, building relationships with clients and stakeholders, and advocating for positive health and wellbeing outcomes. Candidates should have community-based case management experience, strong communication and organisational skills, empathy, and a current driver's licence with a reliable vehicle and appropriate insurance.
