Silverchain is hiring a full-time permanent Care Partner to support older Australians receiving government-funded aged care services in the Western Adelaide and surrounding region. The role involves managing a caseload, conducting client assessments, developing care plans, monitoring budgets, coordinating services, supporting onboarding and ongoing delivery, building relationships with clients and stakeholders, and advocating for positive health and wellbeing outcomes. The position offers schedule and caseload autonomy, access to clinical and service teams, and training opportunities.
