Description
InfuCare Rx is hiring a Care Transition Coordinator (liaison) in the South New Jersey area to coordinate referrals and discharge planning for patients returning home with home infusion services. The role reviews medical records, verifies benefits, identifies home infusion needs, collaborates with physicians, hospitals, case managers, and other care teams, and ensures timely and safe delivery of medications, supplies, and equipment. The position requires travel within the territory, independent judgment, and a minimum of 3–5 years of experience in infusion therapy, home care, or a hospital-related setting; RN or LPN experience is preferred.
