Description
The Case Associate provides intermediate-level direct care to residents of the Jewish Board’s supported housing programs for adults with serious and persistent mental illness. The role supports independent living, recovery goals, medication management, community-based treatment coordination, advocacy, unit inspections, resident escorts, household assistance, documentation, incident reporting, and roommate meetings. It requires experience working with individuals affected by serious mental illness, substance use disorders, or trauma, along with either an AA degree and two years of community mental health experience or an HS/HSE degree and five years of such experience. The position involves travel to residents’ apartments, community driving, and the ability to lift 25 pounds.
