Description
The Case Associate provides intermediate-level direct care to adults with serious and persistent mental illness in supported housing programs. The role supports residents’ independent living, recovery goals, medication management, community appointments, and household needs; coordinates with treatment providers; conducts unit inspections; documents resident interactions; and participates in meetings, trainings, and events. The position requires experience working with individuals affected by serious mental illness, substance use disorders, or trauma, along with either an AA degree and two years of community mental health experience or an HS/HSE degree and five years of such experience. The role may involve travel, driving an agency van, and lifting 25 pounds.
