Description
The Case Associate functions as a Case Planner in the Preventive Services Department, providing in-home child welfare and family support services in New York City. The role assesses living conditions and family dynamics, develops individualized treatment and action plans, coordinates community resources and referrals, provides case management and counseling, monitors high-risk families, maintains case records, and participates in family team conferences. The position is hybrid, may become 100% in person, and requires a bachelor's degree or five years of relevant case-assistant experience, along with Microsoft Office knowledge and bilingual ability preferred.
