The Altes Pastorat transition facility is hiring a case-based support professional to support young adults with mental illness in a special residential setting. The role involves individual care and support planning, participation goals, needs assessment, communication with funding bodies, crisis and conflict strategies, coordination with medical and legal professionals, and program development. Applicants must hold recognized qualifications in education, nursing, special needs care, social pedagogy, social work, or special needs education, and be willing to work late shifts, weekends, and on-call service. The position starts on 01.04.2027 and is available full-time or part-time from 75% capacity.
