Description
The Program Case Coordinator supports older adults and individuals with rehabilitation, dementia, or intellectual and developmental disabilities by coordinating individualized programs, medical appointments, care needs, admissions, documentation, discharge planning, and follow-up. The role collaborates with participants, families, internal teams, healthcare providers, and community partners to promote continuity of care. It is a full-time day-shift position at the 1 Medical Adult Day Center in Brockton, Massachusetts, with a wage of $24.00 per hour plus a full benefits package.
