Description
The Case Coordinator coordinates multidisciplinary healthcare providers and services to advance patient care, manage admission and continued-stay reviews, determine appropriate care levels, document medical and insurance information, facilitate resource use and discharge, and support clinical documentation and patient advocacy. The role requires a nursing degree, current registration and national license, BLS, Case Management Certification, and either six years of SSMC staff-nurse experience or six years of experience including at least two years in case management.
