VON Canada is hiring a full-time Case Coordinator for its SMILE Program in Ontario. The role provides clients with comprehensive assessments, develops care plans, coordinates community and health resources, manages care budgets, monitors progress, and supports client self-management. It is a hybrid remote position requiring work from home and in the community two to three times per week, with possible travel to Trenton and Kingston. The position requires a health or social services degree or college diploma, at least three years of related experience, assessment competency, community-resource knowledge, and a personal vehicle, driver’s license, and vehicle insurance.
