Description
The Case Coordinator provides psychosocial assessment, treatment, and discharge planning for patients across individual, family, and group modalities. The role coordinates interdisciplinary treatment plans, treatment teams, community resources, payers, and family or collateral contacts; conducts utilization reviews; documents care; evaluates treatment response; and supports program improvement. The position is temporary, scheduled Monday through Friday from 8:00 AM to 4:00 PM with holidays as needed, and requires a master's degree in social work, mental health counseling, or marriage and family therapy, along with relevant licensure or limited permits depending on the service setting.
