Description
The Transition of Care/Discharge Coordinator coordinates and implements transition-of-care and discharge plans for ambulatory patients. The role prioritizes and coordinates care across providers, patients, families, and external vendors to ensure timely discharge, using communication, teamwork, critical thinking, and independent task prioritization. It requires a high school diploma or GED, one year of recent relevant experience, communication and time-management skills, and intermediate PC and word-processing skills. The position is full-time, regular, non-exempt, scheduled for 40 hours across five weekdays, and pays $27.28 to $38.19 per hour.
