Sutter Health is hiring a Transition of Care / Discharge Coordinator for SRMC-Sutter Roseville Med Center. The role coordinates and implements transition-of-care and discharge plans for ambulatory patients, prioritizing and coordinating care across providers, patients, families, and external vendors to ensure timely discharge. The position is full-time, limited-term for 26 weeks, scheduled for 40 hours per week on a variable day schedule with one weekend requirement per month, and pays $30.01 to $42.01 per hour.
