Description
The Care Manager provides person-centered care management for individuals with intellectual and developmental disabilities, developing and monitoring care plans and Individual Support Plans, coordinating comprehensive health and community services, managing care transitions and medication reconciliation, documenting care activities, and supervising up to two care management extenders. The role requires relevant human-service education and experience, state licensing or registration, travel as needed, and the ability to work autonomously and meet quality and healthcare standards.
