Easterseals MORC is hiring a Case Management Coordinator to support individuals with disabilities and mental health needs. The role involves engaging clients empathically, screening for co-occurring disorders, assessing needs, developing stage-appropriate treatment plans, maintaining caseload contact, completing clinical and legal documentation, coordinating community-based services, and advocating for supports and resources. Candidates must be QMHPs under Medicaid Provider Manual Guidelines, have specialized training or one year of relevant experience, and hold a bachelor’s degree in a human services field.
