Description
The Case Management Manager leads a team of four Case Managers operating across Germany and reports to the Management of IC Home 24 GmbH. The role is responsible for acquiring referral sources, maintaining occupancy in shared living communities, managing providers, monitoring admission capacities, coordinating with hospitals, authorities, rehabilitation clinics, and payers, building professional networks, and reporting to management. The position requires several years of sales and ideally case-management experience, out-of-hospital intensive-care networking, provider experience, employee development skills, business-management knowledge, and strong negotiation skills. It is based in Norderstedt, Schleswig-Holstein, with two days of home office per week.
