Life’sWork of Western PA is hiring a Case Manager to provide in-home case management services to children, youth, and families. The role partners with referral sources, families, community providers, and county agencies; conducts assessments and service planning; connects clients with housing, mental health, substance-use, employment, and educational resources; advocates for clients; provides crisis intervention; transports clients; and maintains required documentation in the Horizon system. The position requires a bachelor’s degree in social work or a related field, a valid Pennsylvania driver’s license, automobile access and insurance, and required clearances, with availability for evenings and weekends. Compensation is $45,000–$48,000 annually for bachelor’s-degree holders and $50,000–$55,000 annually for master’s-degree holders, plus a performance bonus and health, dental, and vision insurance.
