Keystone Community Resources is seeking a Case Manager to coordinate care and services for people with developmental disabilities. The role involves developing and facilitating Individual Service and Treatment Plans, coordinating providers and appointments, maintaining client records and documentation, advocating for client safety and welfare, supporting admissions and discharges, responding to crises, providing emergency residential coverage, and serving as a contact for families, agencies, and monitoring visits. The position requires a relevant degree and experience working with people with disabilities, a valid Pennsylvania driver's license, proficiency with Microsoft Office, strong documentation and communication skills, and the physical ability to lift, transfer, drive, and perform other mobility-related tasks.
