Description
The Case Manager will work at an adult outpatient clinic to assess and develop individualized service plans for adults living with serious mental illness, coordinate care with behavioral, medical, and other providers, and support recovery through employment, community activity, independent living, and social network development. The role also provides supportive services, monitors behavioral health services, and connects members and families with resources. Compensation is $20-$25 per hour depending on experience, with medical, dental, vision, retirement, paid time off, and other benefits.
