The Case Manager provides direct care and case management to residents and families at Harbour House Family Center in New Bedford, Massachusetts. Responsibilities include assessments, individual counseling, treatment planning, progress documentation, referrals, discharge and aftercare planning, family engagement, housing and economic stability support, and coordination with community and clinical teams.
