Life’sWork of Western PA is hiring a Case Manager in Monessen, Pennsylvania to provide in-home case management services to children, youth, and families. The role involves assessing needs, creating and updating service plans, coordinating with county agencies and community providers, documenting client interactions in compliance with agency and county standards, transporting clients as needed, and working flexible nontraditional hours including evenings and weekends. The position requires a bachelor’s degree in social work or a related field (master’s preferred), or an associate degree with equivalent experience, along with a Pennsylvania driver’s license, vehicle access, background clearances, and strong communication and computer skills.
