Description
The Case Manager is a qualified registered nurse responsible for developing and overseeing individualized patient-centered care plans, providing case management, coordinating care among patients, families, caregivers, and healthcare providers, documenting OASIS and other required data, administering medications, conducting nursing assessments and triage, and supervising ancillary staff. The role requires current nursing licensure, at least one year of nursing experience, and the ability to work nights, weekends, and holidays as assigned.
