Description
Team Select Home Care is hiring a Case Manager who is a registered nurse responsible for overseeing assigned home health patients and ancillary staff, developing and evaluating individualized care plans, conducting nursing assessments and visits, coordinating multidisciplinary care, administering medications, educating patients and caregivers, maintaining clinical documentation, and participating in quality and compliance activities. The role requires a current state nursing license, at least one year of nursing experience, strong communication and time-management skills, computer proficiency, transportation for required travel, and current CPR clearance.
