Description
Team Select Home Care is hiring a registered nurse Case Manager to oversee assigned home health patients and ancillary staff, develop and evaluate physician-supervised care plans, conduct nursing assessments and visits, coordinate multidisciplinary care, administer medications, educate patients and caregivers, maintain clinical documentation and regulatory compliance, and perform on-call nursing triage. The role requires an active state nursing license, at least one year of nursing experience, current CPR, strong communication and organizational skills, independent judgment, transportation for travel, and the ability to meet physical and health requirements. Compensation is $70,000–$85,000 annually with bonus, plus medical, dental, vision, paid time off, 401(k), and referral benefits.
