Description
The Case Manager supports Supportive Living Services for intellectually disabled clients in Lake County, assisting with service delivery, client rights, employee relations, confidentiality, branch audit readiness, and partnership development. The role is based in Ukiah, California, requires at least one year of management experience in assisted living, Microsoft Office knowledge, reliable transportation, and the ability to travel within the county; it is not remote and follows a Wednesday–Sunday schedule with emergency Monday and/or Tuesday coverage.
