Description
The Case Manager conducts needs assessments, intakes, home visits, apartment inspections, biweekly client interviews, referrals, service planning, crisis intervention, documentation, and coordination with landlords, housing agencies, and public-assistance agencies. The role also supports Section 8, NYCHA, and homeless-housing applications and may report child neglect or abuse. Requirements include a relevant bachelor's degree, at least two years of case-management experience, homeless-population experience, housing knowledge, community-resource familiarity, and strong communication skills; a master's degree is preferred.
