ADAPT Community Network is hiring a Case Manager to develop and implement Individualized Service Plans, provide case management and social services, coordinate residential and community support, supervise Residence Program Specialists, maintain confidential records, and advocate for people with developmental disabilities. The role requires a bachelor's degree in Human Services and at least one year of experience working with people with developmental disabilities, and pays $26.00–$27.00 per hour after probation plus benefits for full-time employees.
