Altrecht is hiring a Case Manager for its Overvecht Area Team to provide recovery-oriented, culturally sensitive outpatient care to clients with severe mental illnesses and complex life issues. The role involves maintaining long-term treatment relationships, conducting home visits and intakes, coordinating multidisciplinary and community care, developing treatment and crisis prevention plans, and responding to changes in mental health. Applicants need a completed bachelor's degree in Nursing or Social Work; a post-bachelor's GGZ practitioner, SPV, or mental health case manager qualification and experience with recovery-oriented and outreach work are advantageous. The position offers a gross monthly salary of €4,000–€5,294 for post-HBO qualifications or €3,560–€4,716 without, based on a 36-hour workweek, plus benefits and development opportunities.
