Description
The Case Manager works with single adult households experiencing homelessness in Montgomery County's eastern and central regions. They provide clinical engagement, resource connection, referrals, and person-centered problem-solving to stabilize housing situations or find alternative solutions. Responsibilities include determining resources, implementing service treatment plans, advocating for participants, identifying program needs, and recommending strategies. The role involves three days a week, some flexibility, and focuses on prevention.
