Description
Centerstone is hiring full-time Case Managers to support mental health and substance use programs in Manatee, Sarasota, and Desoto counties. The role provides wraparound services, outreach, care coordination, counseling, treatment planning, behavioral health assessments, client transportation, and documentation, while collaborating with community partners and stakeholders. A bachelor's degree is required; community-based program case managers who take crisis calls or make home visits must have a valid Florida driver's license, a good driving record, and current auto insurance. The pay range is $16.50 to $23.10, with medical, dental, and vision coverage and other benefits.
