Description
Centerstone is hiring a full-time Case Manager to provide mental health and substance use disorder case management, care coordination, resource linkage, supportive counseling, treatment planning, clinical assessments, documentation, and client transport. The role serves at-risk families and individuals across the Community Action Treatment Team and CORe/Outpatient programs, works with community partners and stakeholders, and requires a bachelor's degree, strong communication and organizational skills, electronic medical record competency, and a valid Florida driver's license for community-based program case managers.
